So you’re ready to allow our team to walk life’s journey with you? That’s excellent, we can’t wait to meet you!
If you are a Regional Center client, we need you to contact your Service Coordinator and tell him or her that you are interested in pursuing services with Independence Plus. Then, call or email us and we will come out to your home to meet you.
We have a structured two-part assessment to go over with you so that we can begin to understand what direction you want to see your life head in and how our team can help you get there. We value input from the people closest to you, so please consider inviting your parents or other family members, and any professionals who work with you regularly to our assessment. The second phase of the assessment is intended to gauge your current ability level in your home or community. We may ask to prepare a meal with you, take the bus together somewhere, or even go out to eat. This phase of our assessment is a wonderful opportunity for you to interview us, and see if you think our team will be a good fit for you.
Following our assessment we will write an Individual Service Plan geared towards addressing your hopes, dreams, and personal challenges. If you are a Regional Center client, this service plan will be sent to your Service Coordinator with a formal request for funding.
